After suffering a stroke, Adobe member David* faced losing custody of his grandson—but with the help of our dedicated support team, David turned his life around and kept his family together.
Daily Struggles After a Sudden Stroke
Life can change in the blink of an eye. For Adobe member David, that moment came when he suffered a stroke. Once active and independent, he found himself struggling to carry out simple daily tasks. His body was weak, his balance was off and his speech and mobility were impaired.
David needed support to re-learn how to carry out tasks with his affected functions, but he had no family members to help and limited income to pay for care. As a result, the quality of his living conditions dropped. His once-tidy living room became cluttered, home surfaces developed layers of dust and grime and the kitchen sink piled high with dirty dishes.
Only it wasn’t just David living in these unsanitary conditions. David had been given full custody of his young grandson, Jamie*, who has special needs. The home wasn’t a safe space for Jamie to live, and—as much as David wished it wasn’t the case—his stroke impaired how well he could care for Jamie’s needs. Life for the pair rapidly went downhill.
Reaching a Breaking Point
The situation reached a breaking point when Jamie innocently wandered out of the house one day while David was resting. He was found close to home at a neighbor’s house, uninjured and unharmed, but the concerned neighbor called Child Protective Services (CPS).
When CPS arrived, they saw the state of the home—a reflection of David’s struggles—and a grandfather who was deemed unwilling to care for Jamie. They threatened to take Jamie away. Only David wasn’t unwilling; he was loving and caring yet unable. He needed help and support.
Fortunately, Adobe’s proprietary risk stratification tool identified David as a high-risk member due to his stroke and responsibility for Jamie, helping our team prioritize the case. A social worker from the Adobe Care Team, stepped in before CPS could remove Jamie from his care.
Adobe’s Support and Community Assistance
Our social worker saw what CPS couldn’t. She saw the bond between David and Jamie and knew that separating the pair would be devastating. She reached out to a local agency that helps families in need who sprang into action to offer support. Volunteers came to David’s home and spent hours scrubbing, repairing and organizing the house.
Meanwhile, the Adobe Care Team performed an in-home risk assessment. They identified potential hazards and worked with David to mitigate them, making the home safer for him and Jamie. Our team also conducted a health risk assessment to create a customized care plan for David. This was critical in understanding David’s needs and ensuring he received the care needed to manage his condition.
Recognizing the impact of socio-economic factors on health and David’s low-income status, our team also provided customized SDOH support identified through the MASLOW™ survey. This survey identified gaps in David’s care and helped connect him with additional resources to address his socioeconomic needs.
New Beginnings: Life After Adobe’s Interventions
When Child Protective Services returned for re-inspection a few weeks later, they were impressed with the transformation. The home was a clean, welcoming space. More importantly, it was a safe and healthy environment for Jamie to thrive under David’s care. Thanks to Adobe’s help and support from the community, David and Jamie could stay together.
David’s story shows just how powerful community support can be. Our commitment to positively impact the lives we touch made a world of difference for David and his grandson. The pair currently live happily together, still receiving help and support from the Adobe Care Team to ensure both are living happily and healthily as a family.
